There are some in Pakistan that remove their own limbs so they can make more money begging. He might see his condition as a blessing.

He looks okay, I bet he gets laid more than you think. He is probably looked at as a blessing or a guardian from beyond. Maybe he is a really spiritual person. Maybe he isn't though and is devoid of spirituality. Who knows. His skull is going to be a collecter item after he goes though.
